To provide flexible and quality residential care services for frail elderlies through “person-centered care” and “holistic care”. To ensure elderlies receive holistic support with physical, mental, social and spiritual care.
服務對象
Service Targets
- 年齡在 65 歲或以上申請人（年齡介乎 60 至 64 歲之間的人士，可基於健康及 / 或社會因素提出申請，但須證實確有需要接受住宿照顧，並須符合入住條件）
Aged 65 or above applicant (persons aged between 60 and 64 may apply if there is a proven need for residential care based on health and/or social grounds, subject to their meeting the admission criteria) - 透過『安老服務統一評估機制』被評為適合入護理安老院或護養院。
Assessed to be suitable for admission to care and Attention Home or Nursing Home under the Standardised Care Need Assessment Mechanism for Elderly Services. - 適合群體生活
Suitable for communal living.

Subsidised Places
可聯絡居所附近的綜合家庭服務中心 / 綜合服務中心、醫務社會服務部、長者地區中心或長者鄰舍中心申請轉介
Approach respective Integrated Family Service Centres/Integrated Service Centres, Medical Social Service Units, District Elderly Community Centres or Neighbourhood Elderly Centres for referrals.

終止服務
Service Termination
服務使用者須於一個月前通知院方終止服務，並辦妥有關手續
